By booking a consultation, enrolling in membership, or otherwise using innate-dfw.com, you agree to these Terms of Service. If you do not agree, please do not use our services. You must be at least 18 years old to become a member.
Innate Wellness DFW is a concierge functional medicine and primary care practice offering hormone optimization (TRT/BHRT), peptide therapy, medical weight loss, longevity and performance testing, corporate and executive health programs, and — through our partner, LifeGenix Institute — access to full-body imaging and regenerative medicine. Mobile IV therapy is provided by our sister brand, Tailored by Innate, a separate business with its own terms and pricing.
Membership is offered in tiers (currently Peptide Plus, Innate Elite, and Innate Platinum), each with a defined scope of clinical care, provider access, and included testing. Membership is cash-pay and covers the clinical relationship — consultations, provider access, protocol design, and ongoing management. Medications, peptides, and IV therapy are billed separately at member pricing. Laboratory work is billed directly to your insurance where applicable. Current rates, inclusions, and any cancellation or refund terms will be provided in writing at enrollment.
Certain protocols — including hormone therapy, peptide therapy, and medical weight loss medications — require a comprehensive baseline evaluation before any prescribing decision. We will not prescribe without appropriate clinical justification, and reserve the right to decline or discontinue a protocol we determine is not clinically appropriate for you. Some medications used in our protocols, including certain peptides, may be compounded and are not independently FDA-approved for the specific indication prescribed; see our Medical Disclaimer.
Please provide as much notice as possible if you need to cancel or reschedule an appointment. Repeated missed appointments without notice may be subject to a fee, as described in your membership agreement. Non-urgent provider communication is handled through the channels specified in your membership tier; for medical emergencies, call 911.
LifeGenix Institute and Tailored by Innate are independent businesses. Referrals to either are made at your request or with your consent, and services provided by them are governed by their own terms, pricing, and policies — not this agreement.
You agree to use this website only for lawful purposes and not to misrepresent your identity or medical information when contacting us. All content on this site — text, images, and the Innate Wellness name and marks — is the property of Innate Wellness DFW and may not be reproduced without permission.
To the fullest extent permitted by law, Innate Wellness DFW is not liable for indirect, incidental, or consequential damages arising from your use of our services or this website. Nothing in these terms limits liability for gross negligence or intentional misconduct where such limitation is not permitted by law.
These terms are governed by the laws of the State of Texas, without regard to conflict-of-law principles. Any dispute arising from these terms will be resolved in the state or federal courts located in Dallas County, Texas, unless otherwise required by applicable law.
We may update these terms from time to time. Continued use of our services after a change constitutes acceptance of the updated terms.
