Innate Wellness DFW is required by law to maintain the privacy of your protected health information ("PHI") and to provide you with this notice describing our legal duties and privacy practices regarding that information. This notice applies to PHI created or received during your care with Innate Wellness DFW.
Any other use or disclosure of your PHI — including for marketing purposes or sale of your information — requires your written authorization, which you may revoke at any time.
We may share PHI with business associates who perform services on our behalf, such as laboratories, our scheduling and billing platform, and — only when you request or consent to a referral — LifeGenix Institute. Business associates are contractually required to protect your information consistent with HIPAA.
We are required by law to maintain the privacy of your health information, provide you with this notice of our legal duties and privacy practices, abide by the terms of this notice, and notify you if a breach occurs that compromises your PHI.
We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current notice will be available at innate-dfw.com and at our office.
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
